Health Division Online Update

In order to fulfill recent state requirements your Tax ID number must be submitted along with your renewal fee. You must send your check for payment via mail by including the bottom portion of the Health Renewal Statement you received pictured below. The check must be issued to the City of Boston and display the permit number, business name and address. Please check that the information listed for your establishment is correct and note any changes. If your capacity has changed, you will need to contact the Health Division. Credit card or cash payments can be made at ISD, Mon - Fri: 8 a.m. - 3:45 p.m.
You will not be issued a permit unless this information is received in the Health Division.
