    S/LBE Certification Application:
    Using this one application, a company may apply for certification as a Small Business Enterprise, Small Local Business Enterprise, Minority Business Enterprise and Woman Business Enterprise. The definitions for each business status appear on page 5 of the application.

    Submit the completed application, along with the required documents from the checklist (page 4 of the application), and the Database Input Sheet to the Small and Local Business Enterprise Office.

    S/LBE Database Input Sheet:
    This 3-page document must be completed and submitted with the application and supporting documents from the checklist to the Small and Local Business Enterprise.

    The Database Input Sheet asks for codes relating to the type of goods and/or services provided by the applicant. This website displays the 3 separate listings of codes required to complete this form. They are Category Codes, North American Industry Classification System (NAICS) Codes, and Commodity Codes. These lists should not be printed out and submitted with the application.

    S/LBE Category Codes:
    This is a list of services that have been assigned code letters. The applicant must select up to three (3) of the codes that best describe the services the company performs, and write the codes in the spaces provided on page 2 of the Database Input Sheet. It is not necessary to print this list and submit it with the application.

    NAICS Code and Short Title List:
    The North American Industry Classification System (NAICS) was created through the cooperation and collaboration of committees from the governments of Canada, Mexico, and the United States. This classification system replaced the Standard Industrial Classification (1987) of the United States.

    The Small Business Administration (SBA) has assigned size standards to the NAICS 2002 codes. The City of Boston Small and Local Business Enterprise Office uses the SBA size standards to determine if an applicant qualifies for Small Business Enterprise certification. The applicant must select up to three (3) of the codes that best describe the services the company performs, and write the codes in the spaces provided on page 2 of the Database Input Sheet. It is not necessary to print this list and submit it with the application.

    Commodity Codes:
    This is a list of commodities, or goods, that have been assigned code numbers. If the applicant is a supplier or distributor, he/she may select up to five (5) codes that best describe the products supplied by the company, and write the codes in the spaces provided on page 2 of the Database Input Sheet. It is not necessary to print this list and submit it with the application.

    City Record Subscription Form:
    This is a subscription form for one-year subscription to a publication called the City Record. If an applicant wishes to receive this publication, he/she should fill out the form and submit it with payment to the City Record Office.
