This form can be used to satisfy the letter of notification required in 527 CMR 10.15(1) (b) (Exception) in the city of Boston only. It can be used for buildings classified as R-2 and containing less than four (4) units or R-3, as defined in 780 CMR.

If the occupancy classification is not as stated above a permit application is required instead of a letter of notification. (General Permit Application Link).

Please remember that 527 CMR 10.15 (1) (b) (Exception) requires at least forty eight (48) hours notification to the head of the fire department prior to work commencing.
