File of Life - wallet-sized form

File of Life is a new program offered by Boston EMS, a division of the Boston Public Health Commission. It was designed to bring all your important medical information to the attention of Emergency Medical Technicians in the case of an emergency.

The File of Life is a red plastic magnetic file folder that attaches to your refrigerator. The file contains vitally important information about you so that emergency medical professionals have quick access to your basic medical information.

Also available for your use is a personal size File of Life, which you can carry in a wallet or purse for lifesaving information outside the home.

This information includes:
  • Medications that you take
  • Allergies you have
  • Your Medical Conditions
  • Your Blood Type
  • Emergency Contact Information
  • Your Physician's Name
  • Your Preferred Hospital
