First Choice Lender Initiative (pdf format) This document lists the commitments made by banks who are First Choice Lenders.


DND Authorization Form (pdf format)   A form that indicates a client's permission for specific organizations to view personal records.

Foreclosure Budget Sheet (pdf format)  A form on which a client's income and living expenses are listed

Foreclosure Client Counseling Survey A form that enables clients to provide the Boston Home Center with feedback on the foreclosure counseling they have received

Foreclosure Prevention Intake Form (pdf format)  A form to be completed by a client in order to provide a councilor with basic information about current living expenses and property

Hardship Letter (pdf format)  A letter used to support a client's application to their mortgage lender requesting a revised payment plan

IRS FORM 982 - Reduction of Tax Attributes Due to Discharge of Indebtedness (and Section 1082 Basis Adjustment (pdf format)  A federal tax form to be completed by a client that indicates a mortgage change or foreclosure

No Income Affadavit (pdf format) A form that is used to indicate that a client has no income from any source

No Child Support Affadavit (pdf format) A form that is used to indicate that a client does not receive child support payments as part of their income  

Registration Application of Vacant/Foreclosed Properties  The Inspectional Services Department's online form to register a property that is vacant, slated to foreclose, or has been foreclosed upon, as required by city ordinance
