Medical Registration Information and Forms

Physician's Registration: M.G.L. Ch. 112, Sec. S8.

  • Any Physician practicing in the City of Boston must register with the City Clerk's Office
  • This is a one-time requirement. The fee is $100.00.

Optometrist's Registration: M.G.L. Ch. 112, Sec. 70.

  • Any Optometrist practicing in the City of Boston must register with the City Clerk's Office.
  • This is a one-time requirement.  The fee is $100.00.

Podiatrist (Chiropodist) Registration: M.G.L. Ch. 112, Sec. 21.

Electrologist's Registration: M.G.L. Ch. 112, Sec. 87 HHH.

  • Any Electrologist practicing in the City of Boston must register with the City Clerk's Office.
  • This is a one-time requirement. The fee is $100.00.

  • New Daycare Business Requirement

    When registering a daycare business in the City of Boston, daycare providers are now required to provide a copy of a valid state issued daycare provider license.
