City Clerk

Dear Business Owner,

Massachusetts state law, Chapter 110, section 5, requires business owners to file a business certificate every four years with their respective city or town clerk. Failure to do so is punishable by a fine of not more than $300 for each month such business is not registered.

You may complete this form via this web site, then print it out in preparation of filing it with this office. Alternatively, you may download the blank form and fill it out manually. Please list the name and address of your business, as well as the name(s) and address(es) of the person(s) or corporation conducting the business. This office will fill out the expiration date, which will be four years from the date that the form is registered with us. Non-residents doing business in Massachusetts also pay a $25.00 extra fee, under the provisions of Chapter 227, section 5A.

The filing fee for this registration is $50.00. Please note that your signature(s) must be notarized before you mail it to us, and make your business check or money order payable to the City of Boston. Our mailing address is as follows:

Office of the City Clerk
Room 601 City Hall
Boston, MA 02201

Please also enclose a self-addressed, stamped envelope and we will return your official copy to you.

Thank you for contacting us about this matter. If you have any further questions concerning the business certificate registration procedure, please inquire with Bart McCauley (635-2686).
